01/13/2003 INITIAL CODE VIOLATION Correction Required 1/13/03, REVISITED SITE, TOOK PICTURES, TALKED WITH JUDITH NEW MANAGER, SHE WAS NOT AWARE OF WHAT WAS GOING ON, SHE CALLED CINDY OLD MANAGER TO GET INFO AND CINDY STATED SHE DID NOT KNOW IF ALL EXHAUST FANS WERE COMPLETED OR NOT, GOT A HOLD OF XAVIER, MAINT MAN ON PROPERTY, WE WALKED BOTH BUILDINGS NORTH AND SOUTH, ON NORTH BUILDING THERE WERE 2 EXHAST FANS ON WEST SIDE OF BUILDING THAT WERE NOT OPERATING, WALKED SOUTH BUILDING AND FOUND APPOX. 12 MORE NOT WORKING, RETURNED TO OFFICE AND THERE WAS DAWN THE DISTRICT MANAGER AND WE TALKED ABOUT THE EXHAUST FANS AND THE PERMIT THAT WAS NOT APPLIED FOR BY BARTS ELECTRIC, THEY REMOVED ALL OLD WIRING AND REPLACED WITH NEW TO ALL FAN MOTORS AND ADDED NEW BOXES ON ROOF, AFTER TALKING TO DAWN SHE ASKED FOR ANOTHER WEEK AND I STATED THAT WE HAVE BEEN DOING THIS FOR ALMOST 3 MONTHS NOW AND I STATED THAT I WILL BE BACK ON THURSDAY AFTER NOON, THEY SHOULD BE ABLE TO FINISH A DOZEN MOTORS BY THEN, THATS 3 DAYS
10/30/2002 INITIAL CODE VIOLATION Correction Required 10/23/02, VISITED SITE, TALKED WITH RESIDENTS, RESIDENTS CALLED IN COMPLAINT DUE TO NO COOLING, AT UNIT RESIDENT DID HAVE FAN IN WINDOW AND CEILING FAN BOTH WERE RUNNING, UNIT WAS WARM, UNIT IS ON 5TH FLOOR, HEAT RISES, AFTER TALKING TO RESIDENTS MANAGEMENT SEEMS TO SHUT DOWN COOLING THIS EARLY EVERY YEAR, WAS ALSO ASK TO VISIT ANOTHER UNIT WHICH IS ON SOUTH SIDE OF BUILDING, THE SOUTH SIDE UNITS GET MORE OF THE SUN AND HEAT, THIS UNIT WAS ALSO HOT NEITHER UNIT WAS IN THE EMERGENCY RANGE WHICH IS 100 DEGREES CLOSE APPOX. 90 TO 95, COMPLEX IS ON BOILER AND CHILLER SYSTEM, ALSO FOUND THAT THE EXHAUST SYSTEM WAS NOT OPERATING, AND SMOKE DETECTOR WAS NOT WORKING IN UNIT 550, WENT TO OFFICE TO TALK TO MANAGEMENT, MANAGER WAS OUT UNTIL AFTERNOON, LET NUMBER FOR RETURN CALL, MANAGER CALLED ME BACK, I ASKED ABOUT COOLING AND WAS REFERED TO GLENDA AT CORPORATE OFFICE, ASKED TO HAVE SMOKE DETECTOR IN UNIT 550 REPAIRED AND CINDY (MANAGER) STATED THAT SHE WOULD HAVE REPAIRED SAME DAY, WILL BE SENDING N.O.V. FOR VENTILATION SYSTEM NOT IN OPERATION, ALSO ENTRY AND EXIT DOORS THAT ARE IN NEED OF REPAIR AND INSTALLATION, 30 DAY FOLLOW UP.
