FUTURE INSPECTIONS WILL REQUIRE A LETTER OF CERTIFICATION THE THE ENGINEER OF RECORD HAS RESOLVED ALL PUNCH LIST ITEMS. IN ADDITION TO THE FOLLOWING ITEMS, TRAFFIC ENGINEERING MAY HAVE ADDITIONAL PUNCH LIST ITEMS PERTAINING TO TRAFFIC SIGNAGE AND STRIPING. 1) PLEASE INSURE THAT THE RIGHT-OF-WAY EXCAVATION PERMIT IS CURRENT BEFORE WORKING ON ANY PUNCH LIST ITEMS. IF THE PERMIT IS EXPIRED, A NEW PERMIT FOR THE P.I.A. IS REQUIRED PRIOR TO ANY REMEDIAL WORK OR RE-INSPECTION OF THE SUBJECT SITE. 2) THE SIDEWALK HAS A BROKEN SQUARE AT STA. +/-10+49.41 17.50'LT. 3) THE EDGE OF THE SAWCUT AT STA. +/-10+13.89, 58.55RT HAS CAUSED THE EXISTING ROADWAY TO FAIL AT THE JOINT. 4) THERE ARE GOUGES IN THE NEW PAVEMENT AT +/-STA. 10+53.89, 17.00RT. 5) ALONG THE NORTH SIDE OF HERMAN'S ROAD JUST EAST OF WILMOT THERE IS RIGHT OF WAY WORK FOR UTILITIES IN PROGRESS. THERE ARE OPEN TRENCHES AS WELL AS PILES OF DIRT IN THE RIGHT OF WAY. 6) THE SIDEWALK AT APPROX. STA. +/- 23+00 17.50'LT HAS NOT BEEN BACKFILLED AFTER THE FORMS WERE REMOVED NOR WAS THE CONCRETE SLAG REMOVED FROM THE BUFFER ZONE. 7) THERE IS A BROKEN SIDEWALK SQUARE AT APPROX. STA. +/- 35+05.79, 17.50'LT. 8) THERE IS A POOL OF TAR ON THE NEW PAVEMENT AT APPROX. STA. 34+70.21 17.00RT. 9) PER YOUR PIA CONTRACT, THE ORIGINAL, MYLAR PLAN SET IS TO BE STAMPED "AS-BUILT" IN LETTERS NO SMALLER THAN ONE QUARTER-INCH HIGH ON EACH SHEET OF PLAN SET AND SEALED BY ENGINEER-OF-RECORD AND SURVEYOR-OF-RECORD (IF APPLICABLE) ON COVER SHEET. |
