03/17/2006 ASSURANCE - DOT PERMITS & CODE Denied THIS OFFICE INSPECTED THE SUBJECT PROJECT ON MARCH 16TH, 2006 AND FOUND THE FOLLOWING ITEMS IN NEED OF ATTENTION. PLEASE CONSIDER THIS LETTER THE PUNCHLIST OF ITEMS TO BE ADDRESSED/RECTIFIED PROIR TO THE ACCEPTANCE OF IMPROVEMENTS IN THE PUBLIC RIGHTS-OF-WAY BY THE CITY OF TUCSON FOR MAINTENANCE AND OPERATION. FUTURE INSPECTIONS WILL REQUIRE A LETTER OF CERTIFICATION THA TTHE ENGINEER OF RECORD HAS RESOVLED ALL PUNCH LIST ITEMS. IN ADDITION TO THE FOLLOWING ITEMS, TRAFFIC ENGINEERING MAY HAVE ADDITIONAL PUNCH LIST ITEMS PERTAINING TO TRAFFIC SIGNAGE AND STRIPING.

1) PLEASE INSURE THAT THE RIGHT-OF-WAY EXCAVATION PERMIT IS CURRENT BEFORE WORKING ON ANY PUNCH LIST ITEMS. IF THE PERMIT SEXPIRES, A NEW PERMIT FOR THE PIA IS REQUIRED PRIOR TO ANY REMEDIAL WORK OR RE-INSPECTION OF TH SUBJECT SITE.

2) NO TEST RESULTS FOR ASPHALT HAS BEEN PROVIDED. PROVIDE TESTING FOR THE ASPHALT ON THIS PROJECT.

3) THE ABC SUBGRADE TEST SHOWS THAT IT DOES NOT MEET MINIMUM CITY STANDARDS. PLEASE PROVIDE NEW TEST RESULTS.

4) THE AGGREGATE FIELD DENSITY TESTS- NUCLEAR METHOD STATES THAT THESE TESTS WERE DONE ON A TURN LANE ON SPEEDWAYL. RESUBMIT TEST WITH CORRECT LOCATION.

5) AT STA. 16+19.67. 59.84LT AND STA 13+27.14, 59.95'LT THE TOP OF EXISTING CURBS HAVE BEEN DAMAGED UPON THE REMOVAL OF CURB REPLACED ON THIS PROJECT.

6) BUS SHELTER HAS NOT BEEN CONSTRUCTED AS NOTE DON PLAN.

7) THE BUFFER ZONE, BETWEEN CURBS AND SIDEWALK, HAS NOT BEE LEVELED AND CONTAINS DEBRIS AND CONSTRUCTION MATERIALS.

8) THREE SIDEFWALK SQUARES AT APPROX. STA 16+05.44 HAS BEEN DAMAGED.

9)PER YOUR PIA CONSTRACT, THE ORIGINAL, MYLAR PLAN SET IS TO BE STAMPED "AS-BUILT" IN LETTERS NO SMALLER THAN ONE QUARTER-INCH HIGH ON EACH SHEET OF PLAN SET AND SEALED BY ENGINEER-OF-RECORD AND SURVEYOR-OF-RECORD (IF APPLICABLE) ON COVER SHEET.
