01/08/2010 ASSURANCE - DSD Correction Required AN INSPECTION OF THE SITE WAS DONE BY ELIZABETH LEIBOLD AND I ON THIS DATE AND THE FOLLOWING ITEMS HAVE BEEN NOTED AS NEEDING TO STILL BE ADDRESSED EITHER THROUGH THE ASSURANCES OR WITH THE APPROVED GRADING PLAN FOR THIS SITE:
- Repair or replace cracked spillway concrete outlet pad.
- Reconstruct failing scupper to the east of address 765, south of central basin.
- Add tactile warning domes to accessible crossing ramps at 2 locations: 1) at corner of street south east of central basin; 2) at top of street that is at southeast sideof project.
- Remove stockpile on lots at cul-de-sac at southwest corner of project.
- Revetate lots at cul-de-sac at southwest corner of project.
- Fill shy space area between curb and sidewalk along entrance road and provide erosion protection that connects from Speedway to area with DG near northeast lots.

05/23/2008 ASSURANCE - DSD Approved THE PREVIOUS NOTED COMMENTS HAVE BEEN COMPLETED AT THIS TIME AND THE SITE CAN BE RELEASED FOR THE ASSURANCES.
03/20/2008 ASSURANCE - DSD Partial Approval MET THE DEVELOPER ON-SITE TO DISCUSS THE REMAINING REPAIRS THAT WERE NOTED IN THE PREVIOUS INSPECTION. THEY WILL COMPLETE ALL OF THE REPAIRS AND INSTALLATIONS PRIOR TO CALLING IN FOR A RE-INSPECTION OF THE SITE. COMPLETION OF THE GRADING PERMIT WILL BE CONSIDERED AT THAT TIME IF ALL ITEMS ARE COMPLETE.
03/17/2008 ASSURANCE - DSD Partial Approval THE SITE WAS CHECKED FOR THE FINAL RELEASE OF THE ASSURANCES AND HAS BEEN FOUND TO BE INCOMPLETE IN THE FOLLOWING AREAS:
1) THE DRIVEWAY LOCATED NORTH OF LOT #30 DOES NOT MEET OUR CURRENT STANDARDS FOR THE CROSS SLOPE IN THE AREA OF THE SIDEWALK. THE CURRENT STANDARDS REQUIRE THAT THE SIDEWALK HAVE A CROSS SLOPE OF NO GREATER THAN 2% TO MEET ADA REQUIREMENTS.
2) THE WHEELCHAIR ACCESS RAMPS AT THE INTERSECTION OF LEUTEA DRIVE AND ENCELIA COURT HAVE BEEN DAMAGED AND WILL NEED TO BE REMOVED AND REPLACED.
3) SURVEY MONUMENTS HAVE NOT BEEN INSTALLED PER THE APPROVED FINAL PLAT, THESE HAVE BEEN NOTED MISSING AT THE FOLLOWING LOCATIONS: ENCELIA COURT INTERSECTION (1 TOTAL), SALVIA DRIVE ADJACENT TO LOTS #55 AND #56, #65, #67, #69, AND #73 (9 TOTAL).
4) ENSURE THAT ALL BOLTS IN THE SCUPPER PLATES THROUGHOUT THE PROJECT HAVE BEEN ISTALLED CORRECTLY, INSTALL ALL MISSING BOLTS.
5) PROVIDE A SET OF AS-BUILT DRAWINGS FOR ALL OF THE CHANGES THAT HAVE BEEN MADE TO THIS SITE SINCE THE ORIGINAL DRAWINGS WERE APPROVED.
6) PRVIDE A CERTIFICATION LETTER FROM THE ENGINEER OF RECORD STATING IF THE SITE WAS CONSTRUCTED AS SHOWN ON THE APPROVED SET OF DRAWINGS.

THERE IS A CONCERN IN REGARDS TO THERE NOT BEING PHYSICAL SEPERATION IN GRADE BETWEEN THE ROADWAY AND THE ADJACENT SIDEWALKS WEST OF LOT #63 TO LOT #73.
12/17/2007 ASSURANCE - DSD Partial Approval THE SITE HAS BEEN CHECKED FOR THE FULL RELEASE AND HAS BEEN FOUND TO BE INCOMPLETE AT THIS TIME FOR THE FOLLOWING REASONS:
1) COMPLETE ALL REMAINING ASSURABLE ITEMS ON THIS SITE OR PROVIDE COMPLETION BOND INFORMATION FOR ALL OF THE REMAINING ITEMS THAT HAVE NOT BEEN COMPLETED AT THIS TIME.
2) PROVIDE A CERTIFICATION LETTER FROM THE ENGINEER OF RECORD FOR ALL OF THE DRAINAGE ITEMS LOCATED WITHIN THIS PROJECT.
3) PROVIDE AS-BUILT DRAWINGS FOR ALL CHANGES THAT HAVE BEEN MADE TO THIS PROJECT ON THE APPROVED GRADING PLANS.
4) REQUEST FOR THE FINAL GRADING INSPECTION FOR THIS SITE, THIS INITIAL INSPECTION NEEDS TO BE COMPLETED PRIOR TO RELEASE OF THE ASSURANCES.
09/07/2007 ASSURANCE - DSD Partial Approval ALL LOTS ARE APPROVED FOR THE INITIAL RELEASE AT THIS TIME.
01/31/2007 ASSURANCE - DSD Partial Approval THIS SITE WAS CHECKED TO SEE WHAT AREAS OF PAVEMENT HAVE BEEN COMPLETED SINCE THE LAST INSPECTION. THE ROADWAY HAS BEEN COMPLETED FULLY FOR THE AREA UPTO LOT #61, ALL OTHER AREAS HAVE ONLY BEEN PARTIALLY COMPLETED AT THIS TIME. THE MAIN PORTION OF THE ROADWAY HAS BEEN CONSTRUCTED BUT IS MISSING THE CURB ON BOTH SIDES. I FEEL THAT THIS AREA IS NOT RELEASABLE UNTIL ALL OF THE ROADWAY ITEMS ARE COMPLETE AND IN PLACE SINCE ACCESS WILL BE LOST DURING THE CONSTRUCTION OF THESE ITEMS.
07/07/2006 ASSURANCE - DSD Partial Approval THIS SITE WAS CHECKED TO SEE WHAT AREAS HAVE BEEN COMPLETED SINCE THE LAST INSPECTION. THE FOLLOWING LOTS HAVE BEEN COMPLETED AT THIS TIME: LOTS #30 - #49, #51 - #61. ALL OTHER LOT RELEASES HIGHER THAN LOT #61 WILL REQUIRE AN ADDITIONAL INSPECTION ONCE THE ROADWAYS AND THE UTILITIES HAVE BEEN CONSTRUCTED.

Streets are paved, signage is completed, and curbing has been installed up to Lot # 65.

The retention/detention basin has been roughed-in and the inlet and outlet structures have been built; however a stockpile of dirt and rock is located in the middle of the basin.

Survey monuments have been installed up to Lot # 62.

All HC ramps are in place from Lots # 30 - 46.

Fire hydrants are in place up to Lot # 62.

Utilities, water and sewer appear to be in place up to Lot # 62.

Lot #61 - No house on lot. No sidewalk or driveway apron in place. No surface drainage structures have been built. A utility riser for electric, phone, and cable is in place.

Lots # 60 and 62 - Same as Lot # 61.

Lot # 59 - All assurance items are in place.

Lot # 58 - Same condition as Lot #61 except some surface drainage structures have been roughed in.

Lot # 57 - Same condition as Lot # 61 except a house is on the lot.

Lots # 54 - 56 - No houses are on the lots. Lots are in the same condition as Lot #61.

Lots # 52 and 53 - Houses have been built on the lots. No sidewalks or driveway aprons on lots. A utility riser for electric, phone, and cable is in place. Some surface drainage structures have been roughed in and retaining walls have been built.

Lot # 51 - Not graded yet.

Lot # 50 - Not graded yet.

Lots # 63 - 73 - Not graded yet.

Lot # 49 - No sidewalk, no driveway apron, some surface drainage structures roughed in. No house on lot.

Lot # 48 - Not graded yet.

Lot # 47 - Same condition as lot # 49.

Lots # 43-46 - All assurance items in place.

Lot # 42 - Lot has been graded, it has no house on it. Same condition as lot #49.
