08/18/2025 COT - ENFC - Code Enforcement - Initial In Violation 606 pm - Called out to site by TFD for fire in kitchen, fire isolated to oven per TFD, no damage to structure. The fire suppression system above the oven failed. The fire sprinkler across from oven activated due to fire in oven. GFCI outelt directly under fire sprinkler found tripped. Attempted resetting GFCI, could not test again. Maint man on site isolated power to this outlet by turning off breaker #10. The outlet directly to north did not trip when tested for GFCI. This outlet was also isolated by breaker #10. I found the outlet side of a yellow extension cord sitting near these outlets, ran into the ceiling. I located the plug side of the extension cord plugged into a GFCI outlet on the central wall of the kitchen. I found the GFCI outlet tripped, likely due to the extension cord was wet. Outlet on the east wall of kitchen next to sink is not GFCI. Outlet on far east (exterior) wall next to handwashing sink is not GFCI. Labeled outlet as must remain off, issued notice of violation for repairs to electrical to manager on site. Pima County Health Department on scene, and Southwest Gas on scene. Gas meter was off by TFD prior to SW Gas arrival. SW Gas locked meter and will test lines prior to reconnecting, there is not a need to require a reconnect permit as there is no known damage to lines inside building. -VM
