08/26/2020 INSP - INITIAL CODE ENFORCMENT BERMUDEZ Approved ON SITE; WITH SENIOR INSPECTOR GGOODMAN FOR ON CALL TRAINING.
08/26/2020 INSP - INITIAL CODE ENFORCMENT GOODMAN Approved VISITED SITE, MET WITH TFD OUTSIDE THE 24 HR MCDONALDS RESTARAUNT, ALL EMPLOYEES WERE ALSO WAITING OUTSIDE. TFD STATED THEY WERE CALLED FOR A ELECTRICAL FIRE, FOUND A ELECTRICAL OUTLET UNDER THE DRIVE THRU SODA MACHINE HAD ARCHED AND CAUSED A SMALL BURN AROUND THE OUTLET. TFD STATED ALL THE LIGHTS, KIOSKS, AND DIFFERENT MACHINES CONTINUED BLINKING ON AND OFF WHEN THEY ARRIVED SO THEY TURNED POWER OFF TO THE INTERIOR SUB PANEL OF THE STORE, BUT LEAVING ALL THE COOK STOVES, DEEP FRYERS, AND WALK IN FREEZERS ON THAT WERE NOT AFFECTED. WE WERE THN INFORMED THAT THE FIRE ACTUALLY HAPPENED AROUND 7:30 TO 8:00 PM WHEN ONE OF THE MANAGERS HAD ARRIVED (TFD CALLED ABOUT 1 AM) AND THE MAINTANENCE ARRIVED AND ISOLATED POWER TO THE BURNED OUTLET. CODE ENFORCEMENT TURNED ALARM OFF TURNED POWER OFF AND BACK ON. COULD NOT GET KIOSKS TO STOP BLINKING SO WE TURNED THE KIOSKS OFF LEFT THE KICKED BREAKER OFF AND RESTORED POWER TO THE STORE. WE ASKED MANAGERS TO GET THE MAINTANENCE PERSON BACK OUT BUT LATER CALLED HIM OFF BECAUSE HE WAS THE PERSON THAT ISOLATED THE POWER TO THE BURNED OUTLET ORIGINALLY. TRIED CONTACTING DIRECTOR OF OPERATIONS TO INFORM HIM OF THE INCIDENT BUT RECEIVED NO ANSWER. A MESSAGE WAS LEFT INFORMING HIM OF OUR FINDINGS AND LEFT CONTACT INFORMATION. BEFORE LEAVING WE ALSO RECEIVED INFORMATION THAT WATER FROM THE ICE MACHINE FOR THE SODAS HAD GOTTEN DOWN IN THE AREA OF THE OUTLET WHICH WAS WHEN THE ACCIDENT HAPPENED. A NOV WAS PREPARED FOR AND ISSUED TO MANAGEMENT. I CONTACTED CITY COMM TO DETERMINE WHEN HEALTH DEPT. WOULD BE COMING OUT, CITY COMM REPLIED NOT UNTIL LATER IN THE MORNING. MANAGER WERE INFORMED, CLEAN UP HAD ALREADY BEGUN. NOV ISSUED FOR PERMITS REQUIRED FOR THE ELECTRICAL SYSTEM.
