05/20/2019 | INSP - INITIAL CODE ENFORCMENT | GOODMAN | Approved | VISITED SITE, MET WITH TFD INVESTIGATORS WHO STATED THERE WAS A KITCHEN FIRE AT THE 4 DEEP FIRES INSIDE THE STRUCTURE AND THE HOOD. ANCILLERY SYSTEM DID GO OFF HOWEVER 3 OF THE 4 NOZZLES RELEASED THE ANSULEX POSSIBLY BECAUSE IT WAS NOT PROPERLY CLEANED. MANAGEMENT STATED THEIR COMPANY WAS OUT TO CLEAN THE STORE APPLIANCES THIS PAST WED 5/15/19, BUT FOR WHAT EVER REASON THE DEEP FRYERS WERE NOT CLEANED AND HAD BEEN RESCHEDULED TO BE CLEANED. MY FINDINGS ARE AS FOLLOWS: >FOUR COMPARTMENT DEEP FRYERS WAS WHERE THE FIRE STARTED. ANCEL SYSTEM DID PUT THE FIRE OUT. >HOOD WAS FIRE DAMAGED ENOUGH THAT THE HOOD WILL NEED TO BE REPLACED. >S/W GAS WAS ON SITE, STATED THAT THEY WOULD BE TURNING THE GAS OFF TO THE STRUCTURE AND IT COULD NOT BE TURNED BACK ON UNTIL THE FRYERS WERE REPLACED AND FIRE REINSPECTED THE UNITS AND HOOD. >MCDONALD'S ALSO OWNED THE BUILDING WHICH INCLUDED 3 OTHER STORES IN THE COMPLEX BUT WERE NOT PRESENTLY OCCUPIED. TEP TURNED POWER OFF AT THE 3 PHASE SYSTEM BUT DID NOT CUT POWER. AFTER TEP SPOKE WITH THE OWNER/MANAGER JASON OSBORNE IT WAS DETERMINED THAT JASON KNEW ENOUGH ABOUT THE 3 PANELS INSIDE THE MCDONALDS STORE AND THE PANELS WERE VERY WELL MARKED, THAT POWER COULD EASILY BE RESTORED T THE STRUCTURE. SO BEFORE RESTORING POWER AT THE 3 PHASE PANELS WE TURNED ALL POWER OFF TO THE MCDONALD'S STORE. FIRST RESTORED POWER TO THE 3 ABUTTING STORES. WITH THE HELP OF JASON WE FIRST RESTORED POWER TO THE SITTING AREA LIGHT AND OUTLETS. THEN TURNED POWER BACK ON TO THE FREEZERS AND REFRIGERATORS, THEN ALL EXTERIOR LIGHTS UNTIL EVERY THING HAD BEEN RESTORED EXCEPT ANYTHING CONCERNING POWER TO THE OVENS, FRYERS, AND GRILLES. BREAKERS WERE TAPED, MARKED AND LEFT OFF. >THERE WAS NO STRUCTURAL DAMAGE >WORKERS WERE THEN ALLOWED TO REENTER THE BUILDING AND BEGIN CLEAN UP. >HEALTH DEPT WAS CONTACTED AND ARRIVED ON SITE, WENT OVER WITH THE OWNER OF EVERYTHING SHE WANTED THROWN OUT, AND INVENTORY WAS BEING TAKEN OF ALL FOOD ITEMS THAT THEY WERE THROWING OUT. NOV WAS ISSUED BY HEALTH DEPT. >NOV WAS ISSUED TO REMOVE THE EXISTING FRYERS AND HOOD SYSTEM DUE TO THE EXTENT OF FIRE DAMAGE AND REPLACE UNDER PERMIT. EXPLAINED GAS RECONNECT WOULD BE REQUIRED AND FIRE WOULD ALSO NEED TO INSPECT BEFORE GAS COULD BE TURNED ON. EXPLAINED DSD WOULD CONDUCT ALL OTHER INSPECTIONS ON THE HOOD. THERE WAS NO DAMAGE TO THE BUILDING. NOV SIGNED FOR BY THE OWNER MICHAEL OSBORNE. |
