07/01/2009 INSP - FULL NSPRENGE Violation The Tucson Fire Department has conducted a fire safety inspection at the above stated business . Below is a list of violations that shall be corrected to provide compliance with the applicable codes, regulations and ordinances. Compliance is required by the 2006 International Fire Code (with modifications). 1.__Problem: Your lock box has missing a face plate and is unuseable. TFC 506.2 Key box maintenance. The operator of the building shall immediately notify the fire code official and provide the new key when a lock is changed or rekeyed. The key to such lock shall be secured in the key box. CORRECTION: You will have the face plate replaced and provide keys for access to the sprinkler room and building. 2.__Problem: There is no sign on your sprinkler room door. TFC 510.1 Identification.Rooms containing controls for air-conditioning systems, sprinkler risers and valves, or other fire detection, suppression or control elements shall be identified for the use of the fire department. Approved signs required to identify fire protection equipment and equipment location, shall be constructed of durable materials, permanently installed and readily visible. CORRECTION: You will post a sign on your sprinkler room door that says "Sprinkler Riser Room". 3.__Problem: There are boxes and stored items blocking access to your sprinkler riser. TFC 315.2.3 Equipment rooms. Combustible material shall not be stored in boiler rooms, mechanical rooms or electrical equipment rooms. CORRECTION: You will remove all items stored in sprinkler riser room. 4.__Problem: There is an empty propane canaster stored loose in the sprinkler room. TFC 315.2.3 Equipment rooms. Combustible material shall not be stored in boiler rooms, mechanical rooms or electrical equipment rooms. TFC 3003.5.3 Securing compressed gas containers, cylinders and tanks. Compressed gas containers, cylinders and tanks shall be secured to prevent falling caused by contact, vibration or seismic activity. CORRECTION: You will remove the propane canister from the sprinkler riser room and secure it properly for removal and away from electrical, mechanical, or other buidling service areas. 5.__Problem: There are ceiling tiles missing through out much of the store and breaks in the fire resistant barrier. TFC 703.1 Maintenance. The required fire-resistance rating of fire-resistance-rated construction shall be maintained. CORRECTION: 1) You will replace the missing tiles in: IT storage, floral storage, Over wine rack in wine aisle, breaker panel room, hallwary near breaker panel room, break room, training room, over ice cooler at entrance to store. 2) You will replace missing wall cover over the meat department in storage hallway, 3) You will use fire proof foam to fill gaps around plumbing near the ceiling at the East entrance to the stock room. 6.__Problem: There are cover plates ( blanks) missing over circuits in your breaker boxes. TFC 605.6 Unapproved conditions. Open junction boxes and open-wiring splices shall be prohibited. Approved covers shall be provided for all switch and electrical outlet boxes. CORRECTION: You will place blanks in all breaker panels that are missing a circuit breaker and are open sockets ( as follows): In East Entrance to stock room circuit # 35, in breaker panel room ( #'s CL-3, BA-2, and panel next to EM). 7.__Problem: There are unchained compressed gas cylinders in your store. TFC 3003.5.3 Securing compressed gas containers, cylinders and tanks. Compressed gas containers, cylinders and tanks shall be secured to prevent falling caused by contact, vibration or seismic activity. CORRECTION: You will secure your cylinders : empty CO2 in back stock room, anywhere else they are unsecured. 8.__Problem: There is a discharged fire extinguisher on the floor under the counter in your ICC office. TFC 906.7 Hangers and brackets. Hand-held portable fire extinguishers, not housed in cabinets, shall be installed on the hangers or brackets supplied. Hangers or brackets shall be securely anchored to the mounting surface in accordance with the manufacturer's installation instructions. TFC 906.2 General requirements. Portable fire extinguishers shall be maintained in accordance with this section and NFPA 10. CORRECTION: You will recharge and mount your extinguisher or have it removed from the building. 9.__Problem: There is an extension cord in use. TFC 605.5 Extension cords. Extension cords and flexible cords shall not be a substitute for permanent wiring. Extension cords and flexible cords shall not be affixed to structures, extended through walls, ceilings or floors, or under doors or floor coverings, nor shall such cords be subject to environmental damage or physical impact. CORRECTION: You will remove the extension cord from use. 10.__Problem: There are combustibles stored too close to your hot water heater. TFC 315.2.3 Equipment rooms. Combustible material shall not be stored in boiler rooms, mechanical rooms or electrical equipment rooms. CORRECTION: You will remove the combustibles around your hotwater heater in your back stock room. 11.__Problem: You are missing several escussion plates ( between sprinkler heads and ceiling) through out your store. TFC 901.6 Inspection, testing and maintenance. Fire detection, alarm and extinguishing systems shall be maintained in an operative condition at all times, and shall be replaced or repaired where defective. CORRECTION: You will replace the missing excussion plates: in your cleaning closet, in GM room , break room, where ever else indicated in your sprinkler system inspection report. 12.__Problem: You have wood pallets stored in your 'motor room'. TFC 315.2.3 Equipment rooms. Combustible material shall not be stored in boiler rooms, mechanical rooms or electrical equipment rooms. TFC 304.1 Waste accumulation prohibited. Combustible waste material creating a fire hazard shall not be allowed to accumulate in buildings or structures or upon premises. CORRECTION: You will remove the wooden pallets from the building. 13.__Problem: Your electric panels are blocked. TFC 605.3Working space and clearance. A working space of not less than 30 inches (762 mm) in width, 36 inches (914 mm) in depth and 78 inches (1981 mm) in height shall be provided in front of electrical service equipment. CORRECTION: You will maintain a clear path to and around your electrical panels in your stock room hall way near the 'midpoint' exit, FMC office. 14.__Problem: The exit door from your bakery is NOT opening with the normal amount of force required. TFC 1008.1.9 Panic and fire exit hardware. Where panic and fire exit hardware is installed, it shall comply with the following ...The maximum unlatching force shall not exceed 15 pounds (67 N). CORRECTION: You will have the door serviced so that it opens with 15 lbs or less of force and does not stick. 15.__Problem: There is an unlit exit sign near your bakery on your wine aisle. TFC 1011.4 Exit signs shall be illuminated at all times. CORRECTION: You will cause your exit sign to light on AC Power and battery back up. 16.__Problem: The exit in the bakery storage room is completely blocked with furnishings and merchandise. TFC 1028.3 Obstructions. A means of egress shall be free from obstructions that would prevent its use. THIS IS A LIFE SAFETY ISSUE THAT WILL BE CORRECTED IN 48 HRS OR LESS. ( Due to this exit being from a storage/equipment area, I will allow 48 hrs time to clear a path.) 17.__Problem: The storage in your deli stock closet is to the ceiling and preventing fire streams from reaching fuel in the event of a fire. TFC 315.2.1 Ceiling clearance. Storage shall be maintained a minimum of 18 inches (457 mm) below sprinkler head deflectors in sprinklered areas of buildings. CORRECTION: You will move your storage 18 inches below sprinkler heads. 18. __ Problem: The power to your Hood #1 on your donut fryer did not shut off when your system had its bi-annual test. This is written in documentation from Wildcat Fire who tested your system in February of 09. There is no documentation of the system being repared as of yet. TFC 904.3.3 System interlocking. Automatic equipment interlocks with fuel shutoffs, ventilation controls, door closers, windowshutters, conveyor openings, smoke and heat vents, and other features necessary for proper operation of the fire-extinguishing system shall be provided as required by the design and installation standard utilized for the hazard. CORRECTION: You will have that hood system repaired by a qualified individual or company and tested again to show that your power shuts off upon actuation of the system. You will provide the fire inspector with documentation that states it has been repaired and that the hood is now operating properly. 19. __ Problem: Deficiencies were found in your annual Sprinkler Inspection Report by Wildcat Fire. Since the inspection on 2/17/09, there is no documentation that the deficiencies have been corrected. 901.4.1 Required fire protection systems. Fire protection systems required by this code or the International Building Code shall be installed, repaired, operated, tested and maintained in accordance with this code. CORRECTION: You will have your sprinkler system deficiencies repaired by a qualified individual or company and provide paperwork of such to the fire inspector.
