IF CORRECTIONS HAVE BEEN DONE PLEASE FORWARD A COPY OF THE REPAIR MANIFEST TO THIS OFFICE 1) PROVIDE IDENTIFICATION SIGNAGE FOR THE ALARM TEST LINE AND CONTROL VALVE 2) REPLACE THE MISSING CAP FOR THE FIRE DEPARTMENT CONNECTION 3) SUITE # 3940 - IF THE SPRINKLER HEAD IN THE RESTROOM IS LEAKING IT SHALL BE CHANGED OUT 4) THE SPARE HEAD BOX REQUIRES A WRENCH TO FIT THE CENTRAL BRAND SPRINKLERS ALSO CENTRAL MODEL A 1/2 ", 165 DEGREE PENDANT HEAD SHALL BE PROVIDED 5) SUITE # 3942 - THERE ARE SPRINKLERS ONLY 2' APART, THE REQUIRED DISTANCE IS 6' 6) SUITE # 3940 - IN AN AREA IN THE BACK ROOM A WALL HAS BEEN ADDED, CORRECT SPRINKLER COVERAGE IS REQUIRED 7) SUITE # 3942 - THERE IS A MIXTURE OF TWO DIFFERENT DEGREE SPRINKLER HEADS. THEY SHALL BE CHANGED OUT TO THE CORRECT DEGREE 8) THE SUPPLY SIDE GUAGE AT THE RISER IS INOPERABLE 9) THE ALARM DRAIN AND MAIN DRAIN CONTINUALLY DRIP, THIS SHALL BE REPAIRED AND THE ALARM VALVE LEFT IN THE CORRECT POSITION
