Fire detection, alarm and extinguishing systems shall be maintained in an operative condition at all times, and shall be replaced or repaired where defective. Nonrequired fire protection systems and equipment shall be inspected, tested and maintained or removed. TFC CODE REF: 906.7 Hangers and brackets. Hand-held portable fire extinguishers, not housed in cabinets, shall be installed on the hangers or brackets supplied. Hangers or brackets shall be securely anchored to the mounting surface in accordance with the manufacturer’s installation instructions. REQUIRED CORRECTION: Fire extinguishers shall be serviced annually. Fire extinguishers need to be mounted on brackets. PROBLEM: Emergency backup lights on SE corner of main floor, in VIP room and near women's restroom inoperable. TFC CODE REF: 1006.1 Illumination required. The means of egress, including the exit discharge, shall be illuminated at all times the building space served by the means of egress is occupied. REQUIRED CORRECTION: All emergency backup lighting units shall function properly. PROBLEM: Slide bolt on double doors with panic hardware on main exit, inside bolt lock in office, panic hardware not functioning on Right door in back hallway by women's restroom, bolt lock with panic hardware on south door, In Larry's office door handles lock from outside with key lock from inside. TFC CODE REF: 1008.1.8.4 Bolt locks. Manually operated flush bolts or surface bolts are not permitted. AND: Section 1028.6 Panic hardware. Where panic and fire exit hardware is installed, the panic or fire exit hardware shall be the only locking device on the door except where otherwise permitted in this code. REQUIRED CORRECTION: Slide bolt must be removed from double doors on main entrance. Need thumb turn lock in office, need to make panic harware operable on Right side of double doors in back hallway by women's dressing room. Bolt lock on south door must be removed. PROBLEM: Extension cords used as permanent wiring: Extension cord through wall in VIP room, extension cord behind DJ booth, multi-plug adapter on plug for lights around stage, extension cord behind bar and one through ceiling, extension cord leading from ceiling to T.V. in main area, extension cord outside West eaves? TFC CODE REF: 605.5 Extension cords. Extension cords and flexible cords shall not be a substitute for permanent wiring. Extension cords and flexible cords shall not be affixed to structures, extended through walls, ceilings or floors, or under doors or floor coverings... REQUIRED CORRECTION: Plug appliances into GFI UL rated power strip or directly into electrical outlet. PROBLEM: Cover plates missing from a number of switches and outlets. TFC CODE REF: 605.6 Unapproved conditions. Open junction boxes and open-wiring splices shall be prohibited. Approved covers shall be provided for all switch and electrical outlet boxes. REQUIRED CORRECTION: Switches and electrical outlets shall have cover plates installed. PROBLEM: Open light sockets in phone rooms, wire splicing in open junction box above cashier, open sockets in ceiling in main hall,splicing and electrical cord behind mirror on back wall in main hall,dry storage room has wire hanging through ceiling, janitor's closet in men's room has wiring through ceiling, electrical panel in laundry room needs cover, electrical panels need sign on door outside room and need plugs in open panel switches, wiring in kitchen under phone needs covering, welding room needs plate on electrical outlet and wires coming off light need to be capped off, open light sockets in welding room, maint office wiring needs to be 7 ft or higher or else in conduit, extension cords through walls near main exit out of back rooms and in "Larry's Office", extension cord used as permanent wiring in Larry's office as well as an open junction box. TFC CODE REF: 605.1 Abatement of electrical hazards. Identified electrical hazards shall be abated. AND: Electrical wiring, devices, appliances and other equipment that is modified or damaged and constitutes an electrical shock or fire hazard shall not be used. REQUIRED CORRECTION: Identified electrical hazards shall be corrected by qualified individual in accordance with the electric code. PROBLEM: Holes in ceiling and or wall in main area, walk in storage, in phone room, in step to office, in wall by electrical panel next to bar, in liquor room storage closet and in new liquor storage, dry storage room, janitors closet in men's room, in kitcen, in room off welding room, in "Larry's Office". TFC CODE REF: 703.1 Maintenance. The required fire-resistance rating of fire-resistance-rated construction...shall be maintained. Such elements shall be properly repaired, restored or replaced when damaged, altered, breached or penetrated. Openings made therein for the passage of pipes, electrical conduit, wires, ducts, air transfer openings and holes made for any reason shall be protected with approved methods capable of resisting the passage of smoke and fire. REQUIRED CORRECTION: All holes shall be protected from the passage of smoke and fire. PROBLEM: No exit signs above doors in VIP room, or leading out from women's dressing room hallway, no exit sign leading way out of Larry's office or back rooms. TFC CODE REF: 1011.1 Where required. Exits and exit access doors shall be marked by an approved exit sign readily visible from any direction of egress travel. Access to exits shall be marked by readily visible exit signs in cases where the exit or the path of egress travel is not immediately visible to the occupants. REQUIRED CORRECTION: Exit signs shall be placed above doors in VIP room, in hallway in back room. PROBLEM: Unsecured compressed gas cylinder in welding room. TFC CODE REF: 3003.5 Security. Compressed gas containers, cylinders, tanks and systems shall be secured against accidental dislodgement... REQUIRED CORRECTION: Secure tank in welding room. PROBLEM: Chairs stacked more than 2 feet from ceiling in back storage room. TFC CODE REF: 315.2.1 Ceiling clearance. Storage shall be maintained 2 feet (610 mm) or more below the ceiling in nonsprinklered areas of buildings... REQUIRED CORRECTION: Remove storage from within 2 feet of ceiling. PROBLEM: Exit in closed area blocked with boxes of water, also has a double key lock and no exit sign. This is a required exit. TFC CODE REF: 807.4.4.3 Obstructions. Furnishings or other objects shall not be placed to obstruct exits, access thereto, egress therefrom or visibility thereof. See code references above regarding need for thumb turn lock and exit sign. REQUIRED CORRECTION: Remove boxes blocking exit. Remove double key lock on door and replace with thumb turn lock. Install lighted exit sign above door. Also , as per suggestion of Marc Fagan, remove door leading to blocked exit. PROBLEM: Combustibles stored around base of water heater in laundry room. TFC CODE REF: 305.1 Clearance from ignition sources. Clearance between ignition sources, such as luminaires, heaters, flame-producing devices and combustible materials, shall be maintained in an approved manner. REQUIRED CORRECTION: Remove combustibles from around water heater. PROBLEM: Smoking under canopy on S. side. TFC: 2404.6 Smoking. Smoking shall not be permitted in tents, canopies or membrane structures. Approved “No Smoking” signs shall be conspicuously posted in accordance with Section 310. REQUIRED CORRECTION: Place a clearly visible NO SMOKING sign under canopy. PROBLEM: No certificate of Occupancy on premises. TFC:105.3.5 Posting the permit. Issued permits shall be kept on the premises designated therein at all times and shall be readily available for inspection by the fire code official. REQUIRED CORRECTION: Have Certificate of Occupancy on Premises at all times.
