HAVE SCHEDULED FOR UPGRADE ON SERVICE,TENITIVE FOR THE TWENTY FIFTH OF OCTOBER TWO THOUSAND AND FIVE. VIEWED IN PERMITS PLUS THAT CARMINE HAS APPLIED FOR AN ELECTRICAL PERMIT AND THAT NUMBER IS T05EL02007. CARMINE ALSO STATED THAT HE WOULD KEEP ME POSTED ON STATUS OF UNIT ON A WEEKLY BASIS.

JOESPH STATED THAT HE HAD A ROOM MATE BY THE NAME OF JOHN HORN AND WAS A METH USER. HE RENTED HALF THE COMPLEX FOR FIFTY DOLLARS A MONTH. JOESPH SAID THAT HE HAD JOHN LEAVE TO THE FACT THAT WAS VERY MESSY HOUSE KEEPER AND MADE HIM LEAVE AND THAT JOHN HAD CALLED US IN REPROCUSSION TO DOING SO. FOUND A EXTENSION CORD RUNNING FROM NEIGHBORS RESIDENCE TO HIS UNIT. EXPLAINED TO JOESPH THE HAZZARDS OF BACK FEED FROM ONE UNIT TO ANOTHER AND THAT WE WOULD HAVE TO CONDEM UNIT AND WOULD HAVE TO VACATE RESIDENCE. JOESPH WAS VERY IRRATE AND DEMANDED TO TALK TO OUR SUPERVISOR AND DEMANDED TO KNOW WHY WE WERE CONDEMING UNIT. WAS EXPLAINED TO HIM SEVERAL TIMES ON IMMINENT DANGERS OF BACK FEEDING UNIT. I GAVE JOESPH PHONE NUMBER TO INSPECTOR NUMBER TWO RICK SALDATE(SUPERVISOR). JOESPH CALLED RICK AND DISCUSSED ISSUES. INSPECTED ELECTRICAL SERVICE TO UNITS 1411,1415,MAIN HOUSE. FOUND ONE METER DISCONNECTED TO CENTER UNIT,NONE OF ARE IDENTIFIED. ONE TO THE RIGHT (1411) IS RED CLIPPED AND LACKS SERVICE. CENTER METER BASE (1415) WAS FOUND TO BE JUMPERED, DOUBLE LUGGED ON BOTH SIDES OF DISCONNECT SIDE. LEFT METER BASE FOUND TO HAVE KNOCK OUT MISSING,OLD AND DETERIORATED. ENTERED UNIT 1411,TOOK PICTURES,FOUND CLUTTER IN FRONT OF EXIT DOOR,HOT WATER HEATER AT FLU LAKING TAPE TO SEAL JOINTS,NEW WATER LINES RUN,T.P. VALVE STOPPED BEFORE FLOOR. INSPECTOR J. HURD RECIEVED A CALL FROM RICK SALDATE ON THIS UNIT. INSPECTOR HURD THEN TALKED TO JOESPH THAT WE ARE SHUTTING UNIT DOWN DUE TO LACK OF SERVICE. STARTED INSPECTION AGAIN AND FOUND THAT BATH TUB TILING WAS IN SEVERE DELAPIDATION AND SHEETROCK SHOWED SIGNS OF WATER DAMAGE AND DELAPIDATED. RECIEVED CALL FROM OWNER LAWERENCE CARDAMONE AND TOLD HIM THAT WE ARE SHUTTING UNIT DOWN DUE TO LACK OF SERVICE.OWNER STATED THAT DOES NOT KNOW WHAT TO DO,WAS GOING TO CALL D.S.D. FOR INSPECTION THEN SEE WHAT NEEDED TO BE DONE. ALSO TOLD HIM THAT HE NEEDED TO HIRE A LICENSED ELECTRICIAN TO MAKE REPAIRS ON THE SERVICE ENTRANCES TO ALL UNITS. THEN ASKED HIM WHEN THE REST OF DEBRIS AND VEHICLES WERE GOING TO BE TAKEN CARE OF. ALSO EXPLAINED TO HIM AGAIN THAT HE NEEDED A LICENSED ELECTRICIAN DO A LOAD SURVEY AND SEE WHAT IT WOULD TAKE TO HAVE SERVICE ENTRANCES BROUGHT BACK TO CODE. ALSO TOLD HIM THAT WE NEEDED TO DO AN INSPECTION ON UNIT 1415 ALSO. TOLD LAWERENCE THAT HE ALSO HAD AN OPEN CASE WITH E.S. ABOUT THE DEBRIS AND THE TRASH. INSPECTOR J. HURD GAVE LAWERENCE THE OPPORTUNITY TO FIND A ELECTRICIAN AND TO TO GET BACK TO US IMMEDIATELY AND TO SEE IF HE COULD GET SERVICE BACK ON BEFORE WE HAD TO CONDEM UNIT. EXPLAINED TO JOESPH AGAIN THAT HE NEEDED TO CLEAN RESIDENCE BEFORE WE COULD LET HIM BACK INTO UNIT DUE TO SOME SAFTY ISSUES. CONTACTED MARLENE MORENO FROM PLACEMENT AND DISCUSSED WITH HER ABOUT THE SITUATION AND SHE IS GETTING PACKAGE TOGETHER FOR PLACEMENT OF JOESPH WHEN WE SHUT UNIT DOWN. ELECTRICIAN RODGER FROM DAY STAR ELECTRIC HAD CALLED US AND STATED THAT HE WAS RELAYED FROM OWNER TO COME TO RESIDENCE TO EVALUATE AND SEE IF HE COULD GET POWER RESTORED BEFORE WE HAD TO SHUT UNIT DOWN. RODGER STATED THAT HE WOULD BE THERE IN APPROXIMATELY THIRTY MINUTES. LAWERENCE HAD CALLED BACK AND STATED THAT HE GAVE FULL AUTHORIZATION TO RODGER TO DO WHAT EVER IT TOOK TO RESTORE POWER. CALLED RICK SALDATE AND UPDATED HIM ON SITUATION TO WHAT WE WERE DOING. RODGER SHOWED UP ON SITE AND EVALUATED WHAT IT NEEDED TO BRING SERVICE BACK TO CODE AND STATED THAT HE COULD NOT GET BACK AT LEAST TILL TUESDAY,HIS EVALUATION WAS TO UPGRADE ENTIRE SERVICE. RODGER LARRIVA FROM DAY STAR ELECTRIC STATED THAT THERE WAS NO WAY TO GET ELECTRIC BACK ON TODAY AND IS TO CONTACT OWNER AND DISCUSS WITH HIM WHAT NEEDED TO BE DONE WITH THE SERVICES AND WILL RETURN A CALL TO LET US THE OUTCOME OF WHAT IS TRANSPIRING. CONTACTED MARLENE AND ASKED IF SHE HAD A PLACEMENT FOR JOESPH. MARLENE STATED THAT SHE HAD AND THAT IS GOING TO BE AT THE RODEWAY INN AND IF NEEDED BEYOND MONDAY THAT SHE ALSO HAD THAT SET UP WITH TRAVELERS AID. LAWERENCE HAD CALLED JIM AND STATED THAT HE HAD A RESIDENCE AT 516 E. ADAMS AND IS GOING TO PUT JOESPH IN THIS UNIT UNTIL ELECTRIC GETS TURNED BACK ON AT 1411 N. 3RD AVE. INSPECTOR J. HURD HAD THEN DISCUSSED WITH JOESPH THAT HE WAS NOT TO LIVE IN THE CONDEMED UNIT AND NOT TO REINSTALL EXTENSION CORD. HE IS TO ONLY ENTER RESIDENCE,ONLY TO CLEAN UNIT UP AND NOT BE ABLE TO BE IN UNIT AFTER DARK. TALKED TO LAWERENCE AND DISCUSSED WITH HIM THAT THERE ARE OTHER ISSUES AND THAT IT DEALT WITH UPON FURTHER INSPECTIONS IN THESE UNITS. WENT OVER N.O.V. AND EMERGENCY ORDER TO VACATE WITH JOESPH. JOESPH SIGNED N.O.V. ,POSTED ORDER TO VACATE AND N.O.V. ON PREMISES. AGAIN WENT OVER N.O.V. AND ORDER TO VACATE AND ORDER NOT TO ENTER PREMISES AFTER DARK,JOESPH AGREED THAT HE UNDERSTOOD THIS.
